What can I do with MyDMGHealth?

From viewing test results, paying your bill and refilling prescriptions to receiving care remotely with a video or e‑visit, MyDMGHealth offers you easy access to your medical team and health information whenever and wherever you are. MyDMGHealth is accessible on your desktop or mobile web browser or download the free MyDMGHealth app on your smartphone or tablet.

How do I sign up?

Registering for a new MyDMGHealth account can be completed in a few easy steps.

Please note, if you are brand new to DuPage Medical Group and have not seen one of our providers or do not have an upcoming appointment scheduled, you will need to call us at 1-630-469-9200 to create your medical record prior to setting up a MyDMGHealth account.

Step 1: Request an Activation Code
We need some information from you to match or create your patient record with a new MyDMGHealth account. Complete this form to receive an activation code that will allow you to set up your account.

Your activation code will be emailed to you within 72 hours of the completion of your form. Your activation code email will come from donotreply@MyDMGHealthadmin.com.

If you have already scheduled an upcoming appointment, your confirmation email will contain a link to activate your MyDMGHealth account and you can skip this step.

NOTE: Activation codes can only be requested online for patients 18 years of age or older. For minors or to receive access to another adult's MyDMGHealth account, please complete the Proxy Access forms here.

Step 2: Complete Registration
Once you have received your authorization code, complete your account set up here or at the link provided in your authorization code email. You will be asked to:

  • Choose a username and password
  • Confirm your email address and opt-in for text message notifications
  • Accept the MyDMGHealth Terms and Conditions

Step 3: Log In!
You now have access to your online medical records as well as the ability schedule appointment, message your providers and complete video visits.

PLEASE NOTE: A Spanish version of the Sign Up Form is available for download. However, please keep in mind that our MyChart site is in English only and does not currently have a Spanish version. We’re providing Spanish versions of the forms for your convenience during the sign up process.

Pick the form that is right for you from the list below. If you have an Edward and DMG physician, you can select either form to sign up – you do not need to complete two forms. If you plan to set up an account for your family, choose the appropriate “Proxy Access” form to set up access for you and your child or an adult family member. There is no need to submit two forms, just the Proxy access form.

Can I access my family member's MyDMGHealth record?

Yes, this is called “proxy” access. If you would like to obtain access to a family member’s MyDMGHealth account, please complete the appropriate proxy form below based on the patient’s age.

For children under 12, proxy access allows a parent or legal guardian to log into their personal MyDMGHealth account and view information regarding their child. Per Illinois state guidelines, upon a child’s 12th birthday, a parent or legal guardian will still have access to appointment history, immunizations, and messages but will have limited access to the child’s medical record information. Full access can be granted for children between the ages of 12 and 17 using the Teen Proxy form with the signature of the child, parent or legal guardian, and a witness.

Once a patient turns 18, a parent can get access to their account with the patient’s authorization and completion of the adult proxy form. This form can also be used to request proxy access to another adult’s MyDMGHealth account, such as a spouse, partner, or parent. Adult proxy access can be applied online through an active account for immediate access and a paperless experience.
